Team-based health care group to open up to
20 centers, including its first facilities in Atlanta and
expansions in Las Vegas, Houston and across Louisiana
On pace to double the number of Partners in
Primary Care centers – to more than 100 by 2023
Partners in Primary Care announced today the details of its 2021
expansion. Having opened 15 new centers in 2020, plans for 2021
include up to 20 more centers, including entering the Atlanta
market, additional centers in the Houston area and five more in
Louisiana, including Lafayette and the North Shore outside of New
Orleans as well as two new centers in Nevada.
The planned center openings will bring the total number of
centers Partners in Primary Care operates (including its Orlando,
Florida-based Family Physicians Group facilities) to nearly 80
centers. By 2023, Partners in Primary Care expects to operate
approximately 100 centers.
Partners in Primary Care, a payer-agnostic subsidiary of Humana
Inc. (NYSE: HUM), is part of a family of senior-focused, primary
care centers that deliver comprehensive, personalized care to
approximately 57,000 patients from many different Medicare
Advantage health plans and Original Medicare. The centers are often
located in underserved areas that previously offered minimal access
to primary care for seniors.

Operating under an integrated, comprehensive, value-based care
model, Partners in Primary Care offers a unique health care
experience to seniors. All Partners in Primary Care centers are
staffed by board-certified physicians and care teams who have been
specially trained to treat the senior population. Physicians spend
more time with their patients – well more than the 10 to 15 minutes
in most practices – and care teams focus on creating personalized
care plans designed to help patients achieve their best possible
health. This includes helping patients with social, behavioral and
financial needs.
Outcomes from Partners in Primary Care’s approach indicate that
its model is working to improve seniors’ health. Specifically, data
from Partners in Primary Care’s South Carolina centers showed that
in 2019, 30-day hospital readmission rates for patients were almost
60 percent lower than those of the general Medicare population. The
rate of hospital admissions was 186 per 1,000, significantly lower
than the national rate for this age group, which ranges from 233
per 1,000 (65 to 84 years old) to 456 per 1,000 (age 85+).
Despite the pandemic, patients of Partners in Primary Care
centers not only continued their regular health care, but they also
improved upon it as compared to 2019. Based on claims through
October 2020, 87% of Partners in Primary Care patients made one or
more visits to their local center in 2020, compared to 75% in 2019.
In the case of annual wellness visits (AWV), 94% of at-risk
patients had a comprehensive AWV in 2020, up from 89% in 2019.
For those patients unable to visit a center because of the
pandemic, Partners in Primary Care provided virtual visits,
conducting 9,453 virtual visits and 29,383 telephonic visits over
the course of 2020. Partners in Primary Care and Family Physicians
Group added more than 8,000 new patients in value-based care
reimbursement contracts in 2020.
Partners in Primary Care currently operates centers in Kansas,
Louisiana, Missouri, Nevada, North Carolina, South Carolina, and
Texas. Family Physicians Group operates in the Orlando, Fla.
area.
